No it isn't illegal. Schools can ban any sort of device or activity and can inflict any sort of punishment for a violation of said ban. The fancy term for this is "In loco parentis." At some schools getting caught with a cell phone can be grounds for suspension. The bottom line is that you do NOT need a cell phone at school, and the vast majority of the time it is a distraction from the learning process, which is [believe it or not] the reason you're there in the first place. I am sure the school took great care to make students and parents fully aware of this policy, and I am also sure that the $15 goes towards supplies that will enhance your education. Don't get caught with your personal cell phone and it won't be a problem.

Perfectly legal. This question gets asked at least once a week by some underage kid that thinks the rules don't apply to them.

They have a rule, you know about the rule, you are bound by the rule.

The fine is the punishment for breaking the rule. Schools are pretty much free to punish you in any way they like for breaking the rules.
Since you obviously know the rules about not having a phone at school and even the punishment, then I can see no reason this should worry you.
