What should I do if someone instructs me not to call for emergency medical assistance whatsoever?
Let's say that person will obviously die without that care. I am looking for answers regarding U.S. laws; however, it would be interesting to know how other countries handle this situation--please make sure to specify, though.

You can still dial 911. I would. Even if there's a DNR. But, if the person is on Hospice or something, you should contact the Hospice number and not 911. They'll have a nurse come out to take care of things.

Edit: Yes, the one poster is correct. You CANNOT be sued for calling 911, having an ambulance come, etc. even if there's a DNR. If you know nothing of a DNR order, and you start CPR, you still cannot be sued. You should stop once you do find out. Same goes for EMTs, medical personnel, etc. They should do CPR, defibrillators, etc. if they know nothing of a DNR order. But they'll stop once they find out.

I'd call them and let HIM refuse medical treatment in person with the paramedics or doctors. He can get a living will made if he gets that bad.

****FYI-you CANNOT be sued for calling an ambulance for someone with a DNR order (Do Not Resuscitate). The medical staff could be if they did an actual resuscitation, but you'd be fine.***

Call. It is for the hospital to follow the order not to resuscitate, not you.

Why don't they want medical attention? How are you involved in this situation?

I would call for medical attention anyway. If the person wants to refuse treatment when the ambulance gets there ... that's up to them.

Even if there is a "do not resuscitate" order for the individual ... unless you are medical personnel that means nothing to you. Call for medical assistance anyway, if the patient refuses treatment and they die ... you did what you could.

I get the feeling that the medical situation might be tied to a legal situation (my cynical mind works like that). Is it possible that this situation has something to do with an injury that might be hard to explain?

If so, call for medical help anyway! If the injury helps close an open investigation, so be it.

I know of no law requiring a person with no medical qualifications to seek assistance for an injured or ill third party. I don't know of any law that forbids it either. In the end you do whatever you think is right and hope for the best!
