What is the tax form 1040?Is this the W2 form they send at the end of the tax yr so that you can file your tax?

The W2 is what your employer sent, showing how much you made and how much tax was already taken out, so you can file taxes.
The 1040 is the form you use to file your taxes, where you put the info from the W2, and other info, and calculate whether you need to pay more in tax, or you get a refund. Even if you file electronically, you need to fill out and sign the 1040, and keep it for at least 7 years.
The 1040 is a Federal tax return form. Not everyone files a straight 1040, some file a 1040EZ or 1040A. If immigration is requesting the information and you have a copy of your last year's Federal return. take a copy and send it to them (certified mail). That way you have proof that they received it.
