I am guessing that the answer to this question is complicated. We don't know what goes into an attack during war. You don't know the answer and neither do we. We can all guess and have an opinion but that's about it.

There are accidents in all wars...........and I know it does not help the families that have lost a loved one,however some of the incidents have been due to them been given the wrong co-ordinates,get that wrong in a war zone and sadly the above occurs.
it is so easy for us to sit back and judge what we are being told,however we should maybe try and think what are reactions and feelings would be like in that situation,or how the guys that have shot and killed troops they are working along side feel.I don't think they are partying ....and they have to live with that for the rest of their lives.so easy to judge in the comfort of your own home.

Replace Iran with Iraq and I can answer.
US forces are simply not as good as popular opinion would make them out to be. This isn't to say that they aren't dedicated or brave, as I have no doubt they are, but the US army as a professional military organisation is relatively young. Experience in successful counter insurgency operations is lacking so they are learning almost from scratch 'on the job'.
One reply to this question states that the modern soldier is being 'removed' from the act of killing through the long range nature of the equipment. Not true. But they certainly are desensitised through training. Their equipment is superb on the whole and it is not this that is resulting in a high blue on blue rate.
In response to British troops killed through blue on blue incidents due to bombings during the Falklands war, the number is 0. There was a blue on blue incident where 8 casualties were taken however.
As it happens, the American army has, to it's credit, reduced the number of blue on blue incidents over the last 5 years. Nevertheless, the engagement of friendly forces does seem to be a re-occurring coincidence. I personally believe that it is doctrine that is to blame within both the American army and the general public that leads to Blue on Blue fire. The approach seems to dramatise and cloud an issue when precisely the opposite perspective is required. a shoot first ask questions later attitude. A prime example of recent times is the destruction of a British Scimitar ARV and the subsequent leak of the video capured from within the A-10 US aircraft that engaged it. This was not in the heat of battle- the pilots did not know what it was so they destroyed it.
Ultimately, track records show that casualties due to blue on blue fire always happen in wartime. The American's tend to be demonised for this due to past 'mistakes' and the reputation they hold in the international community.

The war is actually with Iraq, the reason there is so much friendly fire with the Americans is due to lack of discipline in their training as their record of friendly fire goes back to WW2. British troops parachuting in Burma were fired on by American troops despite the British being their allies, their attitude appears to be if it moves shoot it!
What makes you think the Americans are so good? Someone has misinformed you!

Greetings. American military is not very good. never has been. and now with the taking away of firearms as much as possible they are even worse. Here where I live they have what they call Young Marine's that train little boys how to be Marines and give their life for their countries leaders. I was a Marine and don't believe Hitler was right with his Hitler Youth groups that did the same thing to the children in his country. Let the children grow up as humans, not try to raise them as mindless automatons.

Actually, the first answer is pretty good
But if you want an answer about Iraq - how about because the insurgents hole up with innocents, just like they intentionally bomb innocents while, supposedly, we work hard to avoid killing extra people when we can.
Actually, everyone kills extra people in war, that's what makes it nasty.

If your so knowledgable on this subject why are you referring to an Iran war when there is no Iran war? People dying happens during a war. Look at WW1 or WW2 or the Korean War and how many people died. This war in IRAQ is nothing compared to that. As a country we are a bunch of soft whiney babies, also the liberals want our country to be defeated, and with the media coverage deaths are in your face. In comparison to every other war ever fought the casualties in this war amount to nothing in comparison.
