Of course she does. The employee has to sign them. There is space at the bottom for your comments. You can say you agree or you don't agree

Depends where you live and where your employer is incorporated. Most legitimate companies that are incorporated in a first world company would have an obligation to show you or make copies of the papers. It is mandatory on a written request (at least in England, New Zealand and Australia anyway)

Long answer short, Yes.

Any disciplinary actions must be read and signed by the employee. This is actually to protect the employer, in case you are terminated and file a wrongful termination lawsuit against them. Having your signature on file on a copy of the disciplinary papers is proof that you know about it.

Any company not requiring a signature is a sitting duck for a wrongful termination lawsuit.
