My friend just found out that when she was born she was a twin but her other twin was adopted because her mom only wanted one child. She is very up set at her parents for just now telling her since she is an adult now. anyways i want to help her find her twin. Where should we start? all we know is her birthday and her birth name, we are not sure of her parents name or of her name if her parents had renamed her. we also know the city and the hospital where she was born.

As an adult, 18, you can get your adoption records.
I was adopted when I was born by my Aunt and Uncle. My biological parents were way too young to take care of me. (my mom was 14 and idk about my "so called biological father").
My parents kept me being adopted a secret for almost 18 years. Until I finally asked them about it... It broke their heart for me to find that I was adopted out.... They cried and said the only reason that they didn't tell me is because they didn't want me to hate them.
I don't hate them, I respect them a whole lot more... and love them... and thank them for giving me a better life than my little brothers have.
I only found out that I was adopted last October... 2007 (when I was 17)
Btw. I always knew my biological parents but I grew up knowing them as my couisns... My little brothers were my little cousins... but now everyone knows and we (my whole family) are closer than ever.

Call the hospital, see if records are available. Maybe put some type of ad in the local newspaper of the town where she was born. Im sure somebody knows where she is......its nice of you to work with her on this.....

Find out what adoption agency your parents used and contact them. They can give her some "non identifying" information first and then instruct her on her options of finding out more. Most states now have registries and where she can submit her name giving "permission" for her info to be released if someone else related to her has also registered (mom, dad, sister). She needs to be cautious, because sometimes you find out the person does not want to be found or isn't what you expected them to be. I had a good experience, but I know people who have not.

Start broad and become more focused.
Make a craigls list ad.
got to web sites.
Go to the city she was born in and look for birth certificates.
Ask the mother what orphanag/agency set up the adoption as they will still ahve files on her. Is she identical or faternal? If she is identical, then you know kinda what you're looking for.
You could tryo putting an ad in the paper with your friends picture in it.

Because information access is different in each state, it would be helpful to know the state in which this took place in order to give more accurate information.

OMG how devastating.
It depends which State she was born and adopted in. A few states allow access to adoptees' birth records in the same way as non-adopted citizens are entitled.
However, many States still treat Adult Adoptees unequally under the law and continue to deny adult adoptees access to their own records of birth; and when we request them we are subjected to humiliation and frustrating treatment by the people holding our own records hostage. We have to beg and plead and submit court petitions showing why we 'deserve' to know our own roots and origins. The whole thing is just undignified.
Anyway,
Some of the 'closed' states have State run reunion registries but most of them have restrictions and therefore don't work. For example if the adoptee was born in one state but adopted in another they would be unable to register; if one of the parties is deceased then obviously, they would be unable to register; or if the a parents has not been recorded on the birth certificate, they would be unable to register and some people simply do not know about the registries because they are not widely publicized.
However, it's worth looking into. Most States also have a sibling registry too.
If you amend your answer to reflect which State, then there are tons of great people here who can give a more accurate answer for you.
In the mean time make sure to register at these two registries - the largest and most successful ones (but by no means the only ones):
